do the following, after starting the game and see player on screen, close the game and go to your gtaiv.exe folder and send to me the following files:
asilog.txt
ScriptHook.txt
ScriptHookDotNet.txt

well, i can see that some errors occurs during the loading of THulk and THulk enemy scripts:
2014-01-01 13:53:45 - Error during construction of script 'THulk':
System.Threading.ThreadAbortException: Thread was being aborted.
to test you can try to temporary remove all .asi scripts except scriptHookDotNet.asi and any other script that is Inside Scripts folder, leaving only Hulk.
other problem that can be happening is a incompatibility with the ASI loader/scripthook used

lol, thats odd, maybe some issue with models happend and the reinstallation solved restoring the .img files
